We looked at the diseases that cause endobronchial lesions and their clinico-radiological and histopathological profiles.

The endoscopic lesions were classified as infiltrations (superficial lesions without a clear border with normal mucosa) or masses (exophytic lesions easily distinguished from the bronchial wall). Each lesion was categorized according to 3 grades reflecting probability of malignancy.

Bronchoscopy revealed a large, Exophytic, Friable lesion in the Left Mainstem Bronchus (nearly occlusive) which was biopsied followed by fine needle aspiration of 3 peribronchial lymph nodes. Tissue exam of the Endobronchial Mass revealed Primary Adenoid Cystic Carcinoma of the Lung. No malignancy was found within the biopsied lymph nodes.

Computed tomography scan confirmed a 6.5X7.0-cm hilar mass with associated volume loss of the left lung. Bronchoscopy demonstrated an exophytic endobronchial tumor in the apical posterior segment of the left upper lobe.
