Giving the radiological appearance and clinical presentation of uncontrolled hypertension pheochromocytoma is the best fitting diagnosis.

The presence of local invasion or metastatic disease are the only ways to establish malignancy of pheochromocytoma. Unfortunately the patient died before undergoing further imaging or investigations.
